Getting help at home is easier than you might think. Here’s how to begin:
Step 1: Contact My Aged Care
Call My Aged Care on 1800 200 422 or visit MyAgedCare.gov.au.
When you call, they’ll ask a few questions about your health, daily activities, and what kind of help you need. You can also mention that you’d like Heart and Home Support to be your preferred provider.
Step 2: Get an Assessment
A trained assessor will visit you at home to discuss your needs and goals. They’ll help determine what services you’re eligible for and what funding support you can receive.
Step 3: Choose Heart and Home Support
Once you receive your approval letter, contact us at Heart and Home Support. We’ll help you understand your plan, choose the right services, and create a care schedule that fits your lifestyle.
Step 4: Start Your Care
Our friendly team will visit you at home to get started — with warmth, respect, and genuine care every step of the way.
