I’m Evelyn, the founder of Heart and Home Support. After three decades dedicated and committed to my career in healthcare, spanning roles - as a nurse, senior manager, and educator. I have seen the struggles and frustrations that families often face in navigating the complex healthcare system, feeling unheard, overwhelmed or lost in the system.
This inspired me to create something different: care coordination that feels personal, simple, and trustworthy. A service that listens, advocates, and always does what it says.
Heart and Home Support exists for seniors and families who want more than just care - they want peace of mind, dignity, and the freedom to make their own choices.
